Online Application Forms
From 28th December 2009, to comply with the European Union Services Directive, certain applications may be made and paid for electronically by submitting online application forms (see below). The forms may be accessed by clicking on the 'Online application form.' 

Application forms in MS Word and PDF formats for premises licences, variations, premises licence transfer, changes of designated premises supervisor, club premises certificate applications, personal licences and temporary events notices etc may be accessed below. For more information and guidance notes for the various applications that can be made under the Licensing Act 2003, please click on the relevant tab in the left hand column of this page.


Application Fees
Please visit the Department for Culture Media & Sport (DCMS) website to find out how much your application will cost you.

For the grant of a premises licence, or the variation of an existing premises licence, you will have to know within which rataeable value band your premises falls. Please visit the Valuation Office Agency website, where you will find out the rateable value by inserting your Postcode in the appropriate field and search for your premises address. When you have done that, you can compare your rateable value against the table of fees (PDF [26.4Kb] opens in new window) to find out the how much your application will cost you.
