Grants monitoring for 2011/12 ACIP programme
Below are the grant monitoring forms for the ACIP Grants Programme 2009-2011 and 2011/12. You should download them to your desktop, complete and save them and return them according to your contract schedule with Culture Liverpool.

(Use form 1 and form 2 only - two year grants)
Please note: You should submit the quarterly monitoring as per your contract schedule dates and then the annual report form by June 2012 for 2011/12 programme.
How to name your file
Before sending it back please rename the file: Organisation name - Quarter (number) - ACIP (Year) 1112. For example:
- Biennial Q3 ACIP 1112

Culture and Wellbeing Grass Roots Grants 2011
Use form 3 and form 4 only - One year grant.
Please note: The figures in the form 4 expenditure template should list all your invoices and receipts (which should be available for our inspection) and this should match the figures given in form 3.

End project date
Your project must finish by 31st May 2012 and this form must be returned by the end of June 2012.
