Physical disability and sensory impairment
The Physical and Sensory Impairment Team offer assessments to Liverpool Residents aged between 18 - 64 years who have a diagnosed physical disability or sensory impairment.
The main aim of the team is to support individuals to remain as independently as possible within their own home and community.

Physical disability and sensory impairment
A Careline advisor will ask for some information, they will need to know your name, your date of birth, address and the name of your GP. You will also be asked specific questions about your disability and how it affects you.
If you are calling on behalf of someone else you will need to be able to provide the information about the person you are calling for.
If your main concern relates to meal preparation customer advisor will discuss with you the option of having frozen meals delivered.
The more detailed information you can give at this point will assist us to deal with your referral quickly.

What happens next?
The information you give will be passed to the Physical Disability & Sensory Impairment Team (PSI team). You will be contacted by a Social Worker from this team (this could take up to four weeks) either by phone or letter to arrange a convenient time to visit.
During this visit an assessment will be carried out which will assess your needs against Liverpool's eligibility criteria.
What is an Assessment?
The Social Worker will go through an assessment form with you and this will take the format of you answering a number of questions. This will then give a clear indication of how you are managing within your home and in the wider community.
Should you meet the criteria for support the Social Worker will discuss what support would be available to meet your needs. This could be by a Direct Payment or a care package being commissioned by the Social Worker and could include services such as assistance with personal care, day care and support in accessing wider community resources.
What should I expect?
Following the assessment if you are eligible for support and a care package is commissioned you will receive a copy of your assessment and a detailed care plan which will outline how your needs are going to be met and by whom.
After 6 weeks of the care package being in place the Social Worker will hold a review to ensure that the package is meeting your assessed needs. Any alterations will be made if necessary.
Should the care package be working well your care package will then be reviewed every 12 months. However should you experience any problems or your circumstances change then you should contact Careline, 233 3800 and request your review is brought forward.
Is there a Charge?
Liverpool City Council has a charging policy for some services and the Social Worker can arrange for you to meet with an advisor who will discuss what your charges should be and in some cases help you make sure that you receive the maximum benefits that you are entitled to help you pay for your services.
Additional Support
For individuals with a Sensory Impairment we may direct you to Merseyside Society for Deaf People (MSDP) or Liverpool Voluntary Services for the Blind (LVSB) for more specialised support.
Liverpool City Council do not offer a housework or shopping only service and again may redirect you to private agencies who offer this support.
