Customer information

Our customers are important to making this process work. We are reliant on customers to store their assessment information safely and to share this with us when 'we' start our assessment.   
 
It is important that customers are aware that their SAP EasyCare paperwork belongs to them! 

You will need to take your SAP paperwork with you to any appointments that relate to your health or care. This applies if you are admitted to hospital or into respite care. 

A leaflet has been designed to guide you through the process. A copy of the leaflet should be provided when you receive your first assessment using SAP.

Not all customers will be receiving the formal Person Held Record (PHR) for storage of documentation - these will only be given to those customer with complex needs. 
 
However the importance of customers sharing their documentation with professionals is the same whether they receive a PHR or a plastic wallet with paperwork in.
