How can I apply for Extra Care Housing accommodation?

Application can be made direct to the Registered Social Landlord or via the Older Peoples Teams which requires a referral / application and Community Care Assessment.  Applicants must be at least 55 years of age and must be in receipt of a care package from the local authority or have been assessed as requiring one.   For further information contact Adult Careline on 233 3019.

Am I / is my relative eligible for a Community Care Assessment?

Yes, if you / your relative: is resident in Liverpool; is over 18 years of age; has a learning disability; has a physical impairment (including sensory); has a drug / alcohol problem; is an older person with physical or mental frailty; is a carer who provides regular and substantive care for a person who may be eligible for a Community Care Assessment.   For further information contact Adult Careline on 233 3019.

Does my relative / the service user need to know that I have referred them to the Portfolio?

Yes, the only exception to this is if the person at the time of referral has no capacity to understand what was happening in their life.   For further information contact Adult Careline on 233 3019.

What Home Care / Domiciliary Care services are available from the Portfolio?

There are 3 types of Home Care / Domiciliary Care provided, they are: in house home care service - provided by home care staff between 8am - 10pm; mobile night service - provided by home care staff who will visit you between 10pm - 7am to provide personal care, make drinks and ensure you are safe and EMI (elderly mentally ill) service
