Who we are.

We are a team who are committed to working positively for the benefit of customers, partners and the residents of Liverpool.  To do this we will become a model of support in the Health and Social Care system by focusing our attention to customer service and innovation around the Government's Personalisation and choice agenda.

Our Purpose:

  1. Provides a single point of access for sourcing long term care and rehabilitation and enablement care  packages on behalf of  Community and Hospital Social Workers

  2. Sources Continuing Health Care Packages on behalf of the Primary Care Trust

  3. Provide data, information, statistics and performance reports for the whole system


Our Aims and Objectives

Liverpool Brokerage Services aims to meet the needs and requirements of its customers through personalisation, brokerage, navigation, signposting and offering guidance and advice.
We act proactively with service providers to achieve timely responses to requests from Assessment and Care Management teams and the PCT for Social Care requests and Continuing Health Care.   We work closely with the Councils Contracts section (social care) and provide information to commissioners about supply and demand for social care and the support service sector.

What we do

  • Source Domiciliary care packages on behalf of Assessment and Care Management teams and Liverpool Primary Care Trust 

  • Conduct Service Level Recommendations in association with Assessment and Care Management teams 

  • Provide a liaison contact point for the Primary Care Trust, Liverpool City Council home care providers, and the Independent and Voluntary sectors 

  • Deliver information around In-house residential home vacancy information
