CAF form
This is a standard form to help practitioners record, and, where appropriate and with consent, share with others. The findings from the assessment in terms that are helpful in working with the family to find a response to unmet needs.
This form will need to be centrally stored with access controlled by the initiating or lead practitioner. The CAF form will also provide additional information should a practitioner conclude that the childs needs are best met by a referral for specialist.
